Heart Prints
Heart Prints is not a traditional hospice program. Any child within our 9-parish service area up to age 19 who is living with a life-limiting or terminal condition is eligible for the program. Heart Prints patients continue to see their pediatricians or specialists and may continue with curative treatments. What Heart Prints does is provide a link to supportive services and other community resources for the children and their families.
Intended Outcomes
· For the patient:
~ Physical comfort
~ A sense of fullness of life
· For family members:
~ Increased confidence in their ability to safely care for the patient
~ Ability to cope with the anxiety and stress related to caring for a child with a life threatening or terminal condition.
Key Areas
· Supplemental education of patients and family members.
· Emotional and spiritual support for parents, siblings, and other family members
· Case management
(i.e., coordination of referrals to community resources)
ALL THIS IS OFFERED AT NO CHARGE TO THE PATIENTS AND THEIR FAMILIES.
